Mobile Personal Services Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) provide nursing care for children, adults, and seniors in the comfort of their own homes.

Our highly skilled team of professional nurses provide a wide range of services which include, but are not limited to, a comprehensive evaluation of your home health needs, managing chronic conditions or aiding in recovery through post-surgical care, skilled observation of a new or unstable health condition, education for you and your caregivers related to your home health care plan of treatment, medications, diet, exercise and home safety.

Our Nurses confer with your physician to develop an individualized plan of care.

Nursing Services Provides (but not limited to):

  • Skilled Observation and Assessment of a New Diagnosis or the Exacerbation of an Existing Diagnosis
  • Wellness checks
  • Wound Care - Acute, Chronic and Post Surgical
  • Diabetic Wellness Program
  • Diabetic Education by Specially Trained Nurses
  • Blood Sugar Monitoring, Teach Self Use of the Glucose Machine and Insulin Self Injections
  • PT/INR Testing 
  • Other Lab Testing Available as Needed
  • Administer Vitamin B-12
  • Assist in Administration and/or Teach Tube Feeding
  • Administer Medication by Injection
  • Medication Teaching For a New or Changed Medication
  • Catheter Insertion and Maintanence
  • Bowel/Bladder Training
  • Medication Management, Proper Diet and Home Exercise Techniques
  • Neurological assessments
  • Administer/Instruct on Nebulizer
  • Teach Administration of Medications

Physical Therapy:

Our Registered Physical Therapists provide comprehensive physical therapy services that can be safely provided in your home, as prescribed by your physician. Our Therapists will assist your physician in evaluating your condition.

The Physical Therapist will offer instruction to you and your caregivers in the proper care and use of physical therapy devices, as well as establish and implement an individualized physical therapy plan of care.

Medical Social Services:

Our Professional Medical Social Workers assist your physician in evaluating you to determine any environmental, psychosocial or financial barriers that may be impeding your ability to appropriately and safely manage your medical condition. The Medical Social Worker will identify and coordinate many available community resources to improve your ability to attain your health care goals.

  • Evaluation of social, economic, environmental and emotional factors that could effect the patientís condition
  • Obtaining needed community services such as Meals On Wheels, chore services, transportation, financial assistance and support groups
  • Patient and family counseling and education
  • Assistance with long-term planning and alternative living arrangements
  • Stress management/coping skills training
  • Helping eligible patients obtain prescription assistance
